Terms and Conditions for Single Follow-Up Appointment with Dr. Prince Before booking a single follow-up appointment with Dr. Prince, please carefully review and agree to the following terms and conditions: 1. Appointment Confirmation: - Booking a single follow-up appointment constitutes your agreement to these terms and conditions. - Your appointment is confirmed upon receipt of payment. 2. Appointment Cancellation: - If an appointment is not scheduled within 3 days of payment, the single follow-up appointment fee is non-refundable. - If the appointment has already been completed, the single follow-up appointment fee is non-refundable. - If you need to cancel or reschedule your appointment, please do so at least 48 hours in advance to avoid a cancellation fee. - Cancellation within 48 hours of the scheduled appointment may result in a cancellation fee. 3. Privacy and Confidentiality: - Your privacy and confidentiality are of utmost importance. All personal and medical information shared during the appointment will be kept strictly confidential, in compliance with applicable privacy laws. 4. Scope of Services: - The single follow-up appointment includes one (1) single follow-up appointment with Dr. Prince, a review of your baby's progress, and any necessary adjustments to the treatment plan. - Any additional tests, procedures, or treatments recommended during the appointment may involve separate costs and will be discussed with you during the consultation. 5. Communication: - You may contact Dr. Prince's team for questions or concerns related to the treatment plan following the appointment during the one-month support period (30 days) via the specified communication channels. - Dr. Prince's team will respond to chat inquiries during designated business hours. 6. Medical Emergencies: - In the case of a medical emergency or urgent situation, please seek immediate medical attention from an appropriate healthcare provider or facility. 7. Changes to Single Follow-Up Appointment: - Dr. Prince reserves the right to adjust the terms and conditions of the single follow-up appointment, including changes to fees or features. Notice of significant changes will be provided in advance. 8. Dispute Resolution: - Any disputes or concerns regarding the appointment or services provided should be addressed directly with Dr. Prince's office through the provided contact information. By booking a single follow-up appointment with Dr. Prince, you acknowledge that you have read, understood, and agreed to these terms and conditions. If you do not agree with these terms, please do not proceed with booking the appointment. We reserve the right to update the terms of the single follow-up appointment at any time, without prior notice. Use of the single follow-up appointment means you accept any future changes.

What happens after you purchase a follow-up subscription?

A quick note about follow-up subscription scheduling:

Once your subscription is activated, you will receive an email from our patient care coordinator at support@cryingcolicandbeyond.com with instructions for scheduling your follow-up appointment, as well as completing your follow-up questionnaire.
 

After your questionnaire has been completed, you will be able to select from the soonest available appointment options the time that works best for you. Most patients are scheduled within a reasonable timeframe (typically around one week), depending on availability.
 

We know how important timely care is, and we will do everything we can to get you scheduled as quickly as possible.
 

Please note: Processing times may vary, particularly for payments submitted before 9am/after 6pm PDT, over the weekend, or on holidays. If you have not received an email from us within a reasonable timeframe (typically around 24 hours), please check your junk/spam folder or reach out to our team.
